Pet Passport Zambia

Zambia requires an Import Permit for entry. Then the Veterinary Certificate is attached to the APHIS Form 7001 and submitted to the USDA for certification if the pet is departing from the United States or Canada. If you are traveling from another country, the instructions will advise you accordingly.

Your Zambia pet passport is verified at the time of order. Select delivery options of email or first class mail. Note that multiple copies can be made from emailed files.
