Pet Passport Taiwan

Taiwan requires an import permit for entry. Your pet must be micro chipped, vaccinated for rabies and given a Blood Titer Test at least 6 months prior to travel. Your veterinarian then completes the proper Veterinary Certificate and attaches the APHIS Form 7001 and Blood Titer Test for certification by the USDA or CFIA if traveling from the United States or Canada. If you are traveling from a different country, the instructions will advise you of the proper procedure.

If you have more than one pet traveling to Taiwan, additional forms can be self printed from the Email Attachment option. If ordering additional forms for Taiwan with First Class Mail Delivery Option, forms will receive a 30% discount.
