Pet Passport Kazakhstan 

Kazakhstan requires a Veterinary Certificate for entry. Your veterinarian will complete the certificate to be attached to the APHIS Form 7001 to be sent to the USDA or CFIA for certification depending on whether you are traveling from the United States or Canada. If you are entering Kazakhstan from a different country, the instructions will advise you of the proper procedure.
