Pet Passport Japan

Japan requires that you file a notification of import form prior to travel to Japan. They also require completed forms A and C. Your pet must have a 15 digit ISO microchip, current rabies vaccination, Blood Titer Test and a certified Veterinary Certificate for Japan. Then the Veterinary Certificate and Blood Titer Test is attached to the APHIS Form 7001 and submitted to the USDA for certification if the pet is departing from the United States or Canada. If you are traveling from another country, the instructions will advise you accordingly.

If you have more than one pet traveling to Japan, additional forms can be self printed from the Email Attachment option. If ordering additional forms for Japan with First Class Mail Delivery Option, forms will receive a 30% discount.
