Israel requires requires that your pet be micro chipped and vaccinated for rabies and have a Blood Titer Test at least 30 days prior to travel. Your veterinarian must then complete the proper Veterinary Certificate and attach the Blood Titer Test for certification by the USDA or CFIA if traveling from the United States or Canada. If you are traveling from a different country, the instructions will advise you of the proper procedure.Your pet passport package includes:

If you have more than one pet traveling to Israel, additional forms can be self printed from the Email Attachment option. If ordering additional forms for Israel with First Class Mail Delivery Option, forms will receive a 30% discount.
