Pet Passport Hawaii

Hawaii requires that your pet be micro chipped and vaccinated for rabies at least 90 days prior to travel. Your veterinarian must then complete the proper Veterinary Certificate and attach the APHIS Form 7001 for certification by the USDA or CFIA if traveling from the United States or Canada. If you are traveling from a different country, the instructions will advise you of the proper procedure. 

Hawaii also requires a Blood Titer Test and that your pet be treated for Echinococcus (ticks and tapeworms).
