Pet Passport France for Unaccompanied Pets

This is the form you will need if you are sending your pet to France unaccompanied (commercial import). You pet must be microchipped and vaccinatedo for entry. Then the Veterinary Certificate is attached to the APHIS Form 7001 and submitted to the USDA for certification if the pet is departing from the United States or Canada. If you are traveling from another country, the instructions will advise you accordingly.

This product includes the APHIS Form 7001 and forms required for commercial import to France.

Additional forms can be self printed from the Email Attachment option. If ordering additional forms for France with Unaccompanied Pets with First Class Mail Delivery Option, forms will receive a 30% discount.
