Pet Passport Donimica 

Dominica requires that you present an Import Application for entry. Your pet must be vaccinated and your veterinarian must complete a Veterinary Certificate for Dominica. Then the Veterinary Certificate is attached to the APHIS Form 7001 and submitted to the USDA for certification if the pet is departing from the United States or Canada. If you are traveling from another country, the instructions will advise you accordingly.
