Pet Passport Czech Republic Commercial Import  

The Czech Republic requires a Commercial Certificate for import. Pets must be vaccinated and then the Veterinary Certificate is attached to the APHIS Form 7001 and submitted to the USDA for certification if the pet is departing from the United States or Canada. If you are traveling from another country, the instructions will advise you accordingly.
