Croatia requires that your pet be micro chipped with a 15 digit ISO Standard 11784 pet microchip. If your pet has a non-ISO microchip, then you need to carry your own microchip scanner. Your pet must be vaccinated for rabies at least 30 days prior to travel and your veterinarian will complete the Veterinary Certificate for Croatia. Then the Veterinary Certificate is attached to the APHIS Form 7001 and submitted to the USDA for certification if the pet is departing from the United States or Canada. If you are traveling from another country, the instructions will advise you accordingly.
