Costa Rica requires that you advise them in advance if your pet is arriving as air cargo. This is not required if the pet is arriving in the cabin of the aircraft.
To enter Costa Rica, your pet be vaccinated for rabies at least 30 days prior to travel and your veterinarian must fill out the Veterinary Certificate for Costa Rica. Then the Veterinary Certificate is attached to the APHIS Form 7001 and submitted to the USDA for certification if the pet is departing from the United States or Canada. If you are traveling from another country, the instructions will advise you accordingly.

Your Costa Rica pet passport forms are verified at the time of order.
Additional forms can be self printed from the Email Attachment option. If ordering additional forms with First Class Mail Delivery Option, forms will receive a 30% discount.
There are no refunds on pet immigration forms unless there is a problem with the information or forms provided.
